Note that the download link is not ACTUALLY a download link, ie if you click and saveAs, you will get a file called Extension_SimpleForms.php when what you really want to do is actually click on the link and then copy the resulting php code directly into a file called SimpleForms.php...

Test Link to see how cool SimpleForms are: Template:Catlist So Far I must conclude that the mediawiki simple forms extension is useless. Most of the examples are broken.
